The Long Term Post Transplant Follow up Clinic was set up in May 2019 to provide support to long term “well” post liver transplant patients.
Currently run by Sarah Coate and Karen Mercer and under the supervision of Professor Heneghan and Dr Suddle, it is part of the transplant Hepatology service. A number of entry criteria need to be met before patients are transferred to this service – for example being three years post transplant, stable graft and kidney functions – and a range of others. Should the patient’s medical situation change, it might be that they are moved from the clinic back into Consultants’ care.
The Long Term Follow Up Clinic is held on Tuesdays and Thursdays from 2pm until 5pm as part of the post-transplant clinic in Liver Outpatients department. Appointments are made via referral from your Doctor or patient request.
The aim of the clinic is not only to focus on liver function and immunosuppression levels but also to ensure patients are managed holistically and are given time to discuss their general health. It assesses and advises on obesity, diet and exercise, hypertension, diabetes, cardiac risk, medication management/compliance and psychological wellbeing.
All blood results are discussed with a Consultant; Sarah and Karen also ensure letters are sent out quickly post appointment so patients (and their GPs) will have feedback from their clinic appointment in a timely manner.
A huge benefit of this clinic is that patients will always be seen by either Sarah or Karen, giving continuity between appointments. Both Coordinators are Nurse Prescribers so can provide you with a prescription for your repeat medication. This is limited to anti-rejection therapy as all other medications should continue to be prescribed via your GP. Patients are provided with a contact telephone number and email address, providing a more personal service.
A national guideline that is important but sometimes missed is the necessity of national screening post-transplant, including screening for bowel, bone, cervix, prostate and skin. The clinic aims to ensure all patients are up to date with national screening to improve long term outcomes post-transplant.
